Introduction and Instructions

This form is a critical tool for our collective action. The information you provide will form the basis of our complaints to the Irish Government, the European Commission, and the European Parliament.

You must be factual, precise, and unemotional in your answers. Think of this not as telling a story, but as providing evidence for a court case. Stick to what was said, what was done, and what you can prove with documents. Your factual account is our most powerful weapon.

All information you provide will be stored securely and confidentially by the nominated Data Manager. It will only be used for the agreed-upon purposes of our group and will be anonymized in any public or legal submissions unless you provide explicit permission otherwise.

Please use a separate form for each distinct incident or hearing date.
